How does the patient transfer process work?
At HealthPartners Direct, we facilitate hospital admissions and emergency department transfers. When you call, you’ll be connected to a specially trained staff member, who will ask you questions about the patient and referring provider. From there, we’ll connect you with a member of our care team. If appropriate, we’ll arrange a safe, timely transfer for the patient.
Necessary patient transfer information
Please have the following information ready when you call HealthPartners Direct so we can connect the referring provider to the relevant services. (Note: Depending on the patient’s diagnosis, we may ask additional triage questions.)
- Caller name
- Callback phone numbers (including direct number to referring provider, if different than the caller)
- Provider first and last name
- Patient demographics (including first name, last name, date of birth and medical record number)
- Diagnosis and reason for transfer
- Isolation precautions
- IV drip medications (other than saline)
